If all articles are not provided at initial submission it will be considered incomplete and will NOT be processed until all articles are received.

WHO SHOULD APPLY FOR A BIRTH CERTIFICATE?
Afghan Citizens that need documentation stating their place of birth.

NOTE: Who is considered an Afghan Citizen, A person born in Afghanistan, parent born in Afghanistan.

1) Birth Certificate Application

2) Provide Hospital Documents (Afghan Birth Certificate)

3) Provide the following information for Applicant:
  • Date of Birth
  • Place of Birth
  • Father's name
  • Mother's name
4) Picture identification indicating Afghan Nationality. (I.e. Passport, Taskera.) If these Identifications are not available, provide affidavits of two Afghans whom hold Afghan Passports to bear witnesses of your naturalization status. Also obtain a copy of their AFGHAN Passport.

5) Provide Copies of:
  • Green Card (Front and Back) or If you are a US Citizen a copy of your US Passport
  • Driver's License or Identification Card.(Front and Back)
  • Social Security Card (Front and Back)
6) Birth Certificate fees:
  • $10.00
(All fees must be paid with MONEY ORDER)

NO CASH OR PERSONAL CHECKS ARE ACCEPTED
